Fort Bend County Social Services provides comprehensive case management services to vulnerable populations living in Fort Bend County.
Clients are assessed to identify needs and to help them create and implement a service plan to ensure positive outcomes. Through advocacy and collaboration, case managers spend a minimum of 6 months to a year to transition individuals and families to self-sustainability.
